How to apply for studentship letter?

Email the duly completed application form to “exam1_foe@sjp.ac.lk” or hand over to the faculty office.

How to apply for Degree Certificate

Degree Certificates will be issued to the graduands who successfully completed all the pre-
requisites as per the by-laws, rules and regulations of the faculty/ university which will be
issued at the Convocation Ceremony.

How to apply for Degree Certificate->>> Download the application from the university
website after the announcement of calling the degree certificates.

1. All the required information mentioned above, should be dully filled and the
relevant payments need to be done to obtain the detail certificate.
2. The students who have library dues need to pay the fine as a bank or online
transaction to the A/C No: 097-1-001-6-2315454 of the People’s Bank,
Gangodawila branch. Please refer the faculty web for more information
related to this.
3. Please refer the http://192.248.32.115/index.php link for the
transcript fees.
4. The scanned copy of the bank slip for the payments/ screenshot of
online transaction should be sent along with this application form to
exam1_foe@sjp.ac.lk.
5. * – Courier facilities have to be arranged by the students.

How to apply for transcript?

Academic Transcripts are issued to the students on their requests. Upon their requests
Academic Transcripts are issued to the institutes/ universities in relation to the
professional and higher educational purposes of the graduands as well. These certificates
will directly be addressed to the relevant authorities as per the request made by the
students via registered post/ courier service (should be arranged by the students). In order
to issue these certificates, students should successfully completed all the pre-requisites as
per the by-laws, rules and regulations of the faculty/ university.

How to apply for Academic Transcript->>> Download the application from the faculty
website. Dully filled applications with paying off all the payments should be emailed to
exam1_foe@sjp.ac.lk.

How to submit medical for end semester examinations?

Submit absence justification form within 14 days after the exam date with the medical reports approved by
the Faculty Medical Officer/ Chief Medical Officer to the faculty office.
